As multiple providers have discovered throughout the East Tennessee area, our team works very hard to promptly and thoroughly provide the highest standard of care for each patient referred to our clinic. Because our sleep laboratories are open every night of the week, we’re able to schedule patients more quickly. We are proud to be a fully accredited member of the American Academy of Sleep Medicine. Sleep History Form: Getting patients scheduled more quickly!
We have created a form that will help us address your patient’s sleep disordered breathing even more quickly. By including the Sleep History form below with any referral, this will help us skip an initial assessment appointment and enter the pre-authorization process for scheduling an in-lab or at-home sleep study. This dramatically increases the speed at which we can address an individual’s sleep disordered breathing. It also saves the individual from making an additional office visit with a copay. Cash Fees for Services
For patients who prefer to pay for our services without using insurance, here is a list of our cash fees for services:
At-home Sleep Evaluation: $150
In-Lab Sleep Evaluation: $600
In Lab Split Study (Diagnostic Evaluation and Titration of the CPAP device): $650
